It is a civic and generative pact that goes beyond a simple assistance program to transform vulnerability and needs into opportunities for community, solidarity, and social innovation.

Casa su Misura is a social welfare service launched as part of the Alliance for the Elderly, in collaboration with the Fondazione Housing Sociale.
The project is aimed at older adults with limited mobility who need to redesign and adapt their living spaces to their changing health, mobility, and independence, with the goal of ensuring accessibility, safety, and full usability of their homes.
The project is carried out in collaboration with Fondazione Housing Sociale and has been supported by Fondazione di Comunità Milano, which has made it possible to offer the service free of charge through the end of 2023.
The service is available inthe Milan area, with visits made directly to the homes of older adults.
The project was offeredfree of charge through December 2023thanks to financial support fromFondazione di Comunità Milano.
Currently, the service is available ata subsidized rateorfree of charge if the family’s socioeconomic circumstances warrant assistance from the Sportelli dell’Alleanza.
Casa su Misuratakes an integrated and personalized approach, designed to enable older adults tocontinue living in their own homes safely, independently, and comfortably, while reducing risks and preventing the loss of independence.
Throughpersonalized counseling, the service helps older adults and their families understand their housing needs and identify the most appropriate solutions for:
improve the accessibility of spaces;
A specialized multidisciplinary team, consisting of an architect and a case manager, conducts a thorough assessment through dedicated meetings andan on-site visit to the home.
At the end of the process, practical recommendations are provided, including anyproposals for structural modifications, to adapt the home to the person’s needs.
The service also supports familiesin accessing other servicesas needed, thereby facilitating continuous and coordinated care.
Casa su Misura makes a difference because it focuses on the place where well-being takes shape every day: the home.
The project does not offer standard solutions, but rather develops personalized plans that allow older adults to remain safely in their own homes, preserving their independence, identity, and quality of life.
Through a multidisciplinary and integrated approach,Casa su Misuraprevents the risk of institutionalization, supports families, and transforms living arrangements into a tool for care and prevention.
